After completing your free consultation, Confident Therapies will email you forms to fill out. Additionally, please provide copies of the following documents to the SLP at your child's first session/Evaluation:

Confident Therapies provides superbills to families which can be submitted to insurance companies for reimbursement. However, this will be at a higher rate as it acts as an insurance claim, and with insurance claims we are required to bill all insurances the same rate, and this applies to the provision of a superbill. Your insurance carrier may request additional documentation from me to support the submitted claims. The rate billed to insurance will be $140.00. Comfort can contact your insurance carrier to determine out-of-network coverage and help you determine if the discounted private pay rate or using insurance is more advantageous.
